Diversion Case Manager

Position Summary
The Diversion Case Manager is a key member of our Diversion Program. This role works directly with individuals and families who are homeless or at risk of homelessness, helping them identify immediate alternate housing options, access relocation assistance, and avoid entry into the homeless services system. The Case Manager provides personalized assessment, intervention, and support through a trauma-informed, client-centered approach. Services may include facilitation of move-in and relocation assistance for households securing new housing.
Key Responsibilities
- Conduct comprehensive diversion and prevention assessments with clients facing a housing crisis.
- Utilize problem-solving conversations to identify safe housing alternatives and other solutions.
- Coordinate and facilitate move-in assistance and relocation support for clients transitioning to new housing when necessary.
- Connect clients to appropriate community resources (financial assistance, legal aid, mediation services, mental health support, employment services, utility assistance, furniture resources, etc.).
- Work with landlords, property managers, family members, and community partners to support housing placements.
- Maintain accurate documentation in the HMIS system and internal databases.
- Collaborate with internal teams and community partners to enhance diversion outcomes.
- Participate in team meetings, trainings, and program development activities.
- Represent the Christian Service Center with professionalism and compassion in all interactions.
Qualifications
- Minimum 2 years of case management or direct service experience, preferably in homelessness prevention, diversion, housing stabilization, or relocation support.
- Demonstrated knowledge of housing systems, community resources, and trauma-informed care principles.
- Experience coordinating move-in assistance, relocation resources, and housing-related financial supports.
- Excellent communication, mediation, and problem-solving skills.
- Strong organizational abilities and attention to detail.
- Ability to maintain confidentiality and appropriate professional boundaries.
- Commitment to serving individuals with empathy and respect, in alignment with the mission and values of the Christian Service Center.
- Proficiency in Microsoft Office and data entry; experience with HMIS a plus.
- Bilingual (English/Spanish or English/Creole) preferred.

Pay: $46,000.00 - $48,000.00 per year
