DSWs must have a valid Driver's License & an insured vehicle they drive. This is a qualification/requirement for any & all shifts!
Shifts include: Full-time and Part-time available_ (7a-3p, 3p-11p, 11p-7a, 7a-7p, Day shifts, Night shifts, weekends, every other weekend, fill-ins (PRN), 20 -50 hours a week)_

· Job tasks include, but are not limited to:
·Assisting clients with daily needs like: cooking for the client/feeding the client, dressing the client , bathing the client, diapering/assisting your client with toileting, in home and in community supervision.
· Completing time sheets and progress notes accurately and on time.
· Administering medication to clients or give medication reminders
· Arrive on time at client's home.
· Attend team meetings as scheduled.
· Follow Plan of Care of client as requested.
· Being a positive role model for all clients.
Summary
As a Direct Service Worker (DSW) in Lafayette and surrounding areas, you will provide essential caregiving support to individuals in various settings, including group homes and assisted living facilities. Your core skills in patient care and assisting with activities of daily living (ADLs) will be vital in enhancing the quality of life for those you serve. With premium skills in medication administration and patient observation, you will ensure the safety and well-being of clients. A valid driver's license is required, as you will travel to provide home and community care, making a meaningful impact in the lives of others.
