What You’ll Do:
- Provide in-home or on-site care at private residences, senior living communities, and long-term care settings
- Manage chronic conditions, preventive care, and mental health needs for an aging population
- Collaborate with interdisciplinary teams to deliver whole-person, senior-focused care
- Build trusted relationships through empathy, consistency, and patient education
- Support care access and equity by removing barriers for vulnerable seniors

The Role:
As a Senior Doc provider, you will:
- Deliver in-home primary care and/or psychiatric care to seniors in private homes, assisted living communities, and skilled nursing facilities.
- Conduct thorough assessments and implement customized care plans to manage chronic conditions and mental health needs.
- Collaborate with interdisciplinary care teams to ensure whole-person care that meets the physical and emotional needs of our patients.
- Build trust through empathy, education, and proactive communication with patients and families.
