Position: Student Support Services/Health Assistant/LPN
Reports to: Health Services Nursing Supervisor and Assistant Superintendent

- 30 hours/week (part-time)
GENERAL SUMMARY OF DUTIES: Works under the direction of TSC Head Nurse, School RN, and the Principal to provide health care needs primarily to TSC students, but can include TSC staff as needed. Care is provided in accordance with TSC policies, procedures, standing orders and Indiana State Laws. If LPN, responsible for reviewing immunizations, writing health care plans, providing health education , facilitating/performing vision screens, recording hearing screens and completing state reports as mandated by state law.
Position Details:
- 180-182 day position that follows the (180) student calendar (plus 1-4 days before and/or after the school year)
- $18.02 per hour (Health Assistant)
- $25.89 per hour (LPN)

Qualifications:
- Certification or Registration as a Medial Assistant or Emergency Medical Technician

- Current Adult, Child, and Infant Cardiopulmonary Resuscitation program certificate
- 3 years of experience as a LPN, preferably in pediatrics, community health nursing, adult, and/or mental health nursing
- High School Diploma
- CPR Certification (required)
- First Aid Certification (required)
- A medical background (desirable)
- Experience and ability to work with children
- Good physical and emotional health
- Proficient in use of computer and knowledgeable with basic computer software. Able to operate standard office equipment, and use a telephone
Physical Requirements:

- Student Care
- First Aid/Emergency Care
- Provides first aid/emergency care for illness or injury occurring in school or while the student is under the jurisdiction of the school
- Informs parents of sick or injured students following the direction of standing orders
- Assists with the head louse program
- Performs interventions as assigned and trained by school nurse
- Organizational Duties
- Completes incident/accident reports – to be reviewed and initialed by the school nurse
- Records screening results and special health needs on individual health cards – to be reviewed and initialed by the school nurse
- Makes out medication sheets – to be reviewed by the school nurse
- Stocks health room supplies in a cost conscious manner
- Maintains Record of Care Provided to Students in Health Office
- Documents care given to students
- Follows TSC policies and procedures as well as the school physician's recommendations and Standing Orders

- Carries out Other Nursing Functions as Assigned by the School Nurse

LPN Additional Performance Responsibilities:
LPN Nursing Care
- Provides acute or chronic health care, first aid, and emergency care to students and staff in accordance with professional scope and standards as identified by the Indiana Board of Nursing and school policy and procedures.
- Administers medication with appropriate documentation and in accordance with the Health Services Clinical Policies.
- Follows Individualized Healthcare Plans as developed by the RN
- Responsible for noting changes in the patient status and recognizing deviations from the established plan of care. Notifies the Head Nurse for further instructions.
- Participates in maintaining accurate medical records to assure compliance with state mandates including immunizations, communicable disease reporting, OSHA, and acute or chronic medical conditions.
- Performs Department of Education recommended screening procedures.
- Reports and/or assists in referrals for suspected abuse/neglect as a mandated reported, this will be conducted in collaboration with school principal.
LPN Communication
- Maintains communication with the Head Nurse, teachers and other school personnel, and parents/guardians to promote a healthy and safe environment for students.
- Assists in providing health services, information, and counseling within the scope of practice, in a cost efficient and effective manner. Notifies the Health Nurse of any student found to have abnormal findings for assessment and referral.
- Completes accident/incident reports for students/staff and communicates with Head Nurse
- Maintains confidentiality regarding school and health-related issues, unless legally mandated to report.
LPN Organization
- Maintains electronic health records in Health Office Anywhere of student visits and documentation on individual health records.
- Maintains the health office facility and supplies.
LPN Resource Utilization
- Utilizes existing EMR – Health Office Anywhere technology effectively in the performance of duties.
- Records 100% of health visits into Health Office Anywhere
- Compiles data for statistical purposes
LPN Professional Development
- Attends nurses’ staff meetings, in-services, and credentialing programs provided by the district
- Participates as an active member of the school community, representing health, wellness and safety
