Position Summary:
The Health, Safety, and Nutrition (HSN) Coordinator is responsible for developing, monitoring, and evaluating health, safety, and nutrition services that ensure children and families receive quality preventive and primary care. This position integrates expertise as a Certified Hearing and Vision Technician and Community Health Worker (CHW) to provide direct screenings, referrals, follow-up, and culturally competent community engagement. The HSN Coordinator supports family access to health resources, ensures compliance with Head Start Program Performance Standards (HSPPS), DCFS licensing, and HBH policies, while actively promoting health equity and safety across all sites.
Duties and Responsibilities:
Health & Safety Oversight
- Develop, monitor, and evaluate health services and family care access to ensure HSPPS and DCFS regulations compliance.
- Conduct site monitoring visits, including file reviews, injury/incident tracking, safety inspections, and emergency preparedness compliance.
- Ensure all facilities maintain safe indoor and outdoor environments, free of hazards.
- Hearing & Vision Technician Duties
- Administer age-appropriate hearing and vision screenings for enrolled children according to state and federal requirements.
- Accurately document screening results in COPA and communicate outcomes to families and staff.
- Provide referrals for follow-up care, track completion, and assist families with securing necessary resources or professional evaluations.
- Maintain screening equipment, calibration, and sterilization logs.
- Community Health Worker Duties
- Serve as a liaison between families and healthcare systems, supporting families in overcoming barriers such as insurance access, transportation, or medical home establishment.
- Conduct culturally responsive outreach, health education, and case management.
- Advocate for families by connecting them with WIC, dental, nutrition, and medical providers, and maintain community resource partnerships through formal MOUs.
- Nutrition & Health Education
- In collaboration with nutrition consultants and site staff, develop and present health and nutrition education to parents.
- Support Individual Education Plans (IEPs) or Individual Service Plans (ISPs) related to nutrition or dietary modifications.
- Follow up on children with identified health and nutrition concerns (e.g., chronic illness, underweight/overweight/obesity) to ensure treatment compliance.
- Family & Staff Support
- Provide technical assistance and training to staff on child health, safety, and nutrition policies.
- Collaborate with Family Support Workers (FSWs) to ensure health referrals and follow-ups are documented and communicated with families.
- Participate in agency and grantee training and professional development, and serve as co-facilitator of the Health Services Advisory Committee (HSAC).
- Documentation & Monitoring
- Ensure complete and accurate documentation in COPA, including health events, screenings, referrals, and follow-ups.
- Prepare reports for the Program Information Report (PIR), ongoing monitoring, and annual reviews.
- Monitor compliance with EPSDT requirements to maintain 90% or higher completion rates.
- Other Duties
- Collaborate with HSN Manager and specialists to update health, sanitation, and food service policies and procedures.
- Perform other duties as assigned.
Qualifications:
- A bachelor’s degree in early childhood development, nursing, nutrition, public health, or a related field from an accredited institution is required.
- Certification as a Hearing and Vision Technician (Illinois Department of Public Health or equivalent).
- Certification as a Community Health Worker (CHW), preferred (or willingness to complete certification within 12 months).
- Knowledge of Head Start Performance Standards, DCFS licensing regulations, and child health best practices.
- Strong analytical, organizational, and communication skills.
- Ability to work effectively with diverse families and community partners.
- Computer proficiency in Microsoft Word, Excel, PowerPoint, Outlook, and the COPA data system.
- Must have daily vehicle access and valid auto insurance.
- Employment requires proof of vaccination or an approved exemption.
Physical Requirements:
The Department of Children and Family Services requires a current physical exam, proof of Hepatitis B immunity, and TB skin test.
Salary
Starting from $40,000 to $60,000 contingent upon educational credentials completed
