Position Summary:
We are seeking a dedicated Home Health Aide to join our In-Patient Facility team. This role is essential in providing personal care and support to patients during the final stages of life, ensuring comfort, dignity, and a safe, clean environment.
Key Responsibilities:
- Provide personal care and hygiene assistance per patient care plan
- Support patients and families with respectful communication and compassionate care
- Maintain a clean and safe environment for patients
- Report any changes in patient condition to RN/Case Manager
- Participate in interdisciplinary team (IDT) meetings
- Follow infection control standards and maintain accurate documentation
Requirements:
- Valid California Home Health Aide License
- Completion of a state-approved Home Health Aide training program
- Current CPR certification
- Must pass a Home Health Aide competency exam
- Reliable transportation
- Ability to work independently and collaboratively
- Must be able to work a minimum of 6 shifts per month (including 2 weekend shifts); flexible with day, PM, or NOC shifts
Preferred Traits:
- Compassionate, dependable, team-oriented
- Strong communication skills
- Experience in hospice or palliative care a plus

Pay: $24.20 - $27.50 per hour
