Job Summary
We are seeking a compassionate and dedicated Home Health Aide to provide essential support and care to individuals in their homes. The ideal candidate will assist clients with daily living activities, ensuring their comfort and safety while promoting their independence. This role requires a strong commitment to patient care and the ability to work effectively in a home environment. CLUSTER CARE SERVICES, SERVICING MULTIPLE PATIENTS A DAY, 1 HR PER PATIENT. HHA can see anywhere between 3-10 patients a day, depending on their own availability. Some patients may be in the same building, as well as buildings with only 1 patient. All patients are seen 7 days a week, an HHA with open availability and a flexible schedule is in need.

Duties
- Assist clients with personal care tasks, including bathing, dressing, grooming, and toileting.
- Provide medication administration as prescribed and monitor clients for any side effects or changes in condition.
- Measure and record vital signs such as blood pressure, temperature, and pulse.
- Prepare nutritious meals according to dietary restrictions or preferences.
- Offer companionship and engage clients in meaningful activities to enhance their quality of life.
- Assist with mobility and transfers to prevent falls and ensure safety.
- Provide specialized care for clients with dementia or memory-related conditions.
- Support clients with catheter care and other medical needs as required.
- Maintain a clean and safe environment for clients by performing light housekeeping duties.
Experience
- Familiarity with patient care techniques, including vital signs monitoring and medication administration.
- Experience in home community care or home health settings is a plus.
- Knowledge of dementia care practices is beneficial.
- Strong interpersonal skills with the ability to communicate effectively with clients, families, and healthcare professionals.
- A caring attitude towards patients and a commitment to providing high-quality care.
