Cor Meum Home Health Care Services is seeking a motivated, outgoing, and results-driven Home Health Liaison to join our growing team. In this role, you will serve as the vital link between our agency and referral sources, including hospitals, physicians, skilled nursing facilities, and community organizations. You will be responsible for promoting our home health services, identifying patient referral opportunities, and driving census growth through strategic outreach and relationship building.
This is a performance-based role with competitive compensation tied to the number of eligible patient referrals.
Key Responsibilities:
- Develop and maintain strong relationships with physicians, discharge planners, case managers, social workers, and other referral sources.
- Identify and secure patient referrals appropriate for home health services.
- Conduct in-person visits, presentations, and service education to promote awareness of our offerings.
- Serve as a liaison between referral sources, patients, families, and the agency to ensure smooth transitions and timely admissions.
- Meet or exceed monthly referral and admission targets.
- Maintain accurate records of sales activities and referral interactions.
- Background in medical or technical sales is a plus.
- Strong communication and presentation skills.
- Organized and detail-oriented.
- Comfortable with product demos and client meetings.
- Able to analyze client needs and suggest solutions.
- Familiarity with outside sales is helpful.
