Position Overview:
As the Home Health Clinical Supervisor, you’ll be responsible for overseeing the daily clinical operations of a skilled nursing and therapy team, ensuring high-quality patient care, compliance with state and federal regulations, and efficient coordination with physicians and case managers.
✅ Key Responsibilities:
- Provide clinical oversight and supervision for field staff (RNs, LVNs, Therapists, HHAs)
- Ensure timely initiation, planning, and coordination of patient care
- Review and approve OASIS documentation, care plans, and visit notes
- Conduct performance evaluations and support staff development
- Participate in quality assurance, compliance, and infection control initiatives
- Serve as a liaison between the agency, physicians, patients, and families
- Participate in on-call rotation as needed
Qualifications:
- Active RN license in the State of Texas (required)
- Minimum 2 years of Home Health experience (required)
- Prior supervisory or leadership experience (preferred)
- Knowledge of OASIS, Medicare guidelines, and EMR systems
- Strong organizational, communication, and leadership skills
