Overview
We are seeking a dynamic and proactive Liaison to facilitate effective communication and collaboration between various departments within our organization. The ideal candidate will play a pivotal role in ensuring that marketing initiatives align with business objectives, driving growth and enhancing our brand presence in the market. This position requires a blend of strategic thinking, creativity, and strong interpersonal skills to successfully manage relationships with internal teams and external partners.
Duties
- Operations: Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns. Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to manager. Monitor Home Health Aide supervisory visits.
- Patient Care: Work in the field with staff and patients as needed.
- Customer Service: Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
- Minimize Patient Risk: Ensure Start of Care compliance and quality and safe delivery of home health services. Focus on Outcome Monitors as assigned. Implement quality improvement programs in the field.
- Policies: Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance/ Company and Hospital guidelines, policies and protocol.
- Teamwork: Assist with monthly Case Manager, Nurse and Therapy meetings. Participate in all on-call requirements, and patient staffing as needed.
Experience
- Develop and execute marketing strategies to promote home health services.
- Identify and build relationships with potential referral sources (physicians, case managers, discharge planners, etc.)
- Conduct regular visits to hospitals, clinics, skilled nursing facilities, and other potential referral sites.
- Educate referral sources about the agency's services and the benefits of home health care.
- Collaborate with the clinical and intake teams to ensure smooth onboarding of referred patients.
