We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team in the Seattle/Tukwila region of WA. The ideal candidate will provide high-quality nursing care to patients in a home setting.

Pay Rate: Range $1680 to 1720 Weekly Gross pay based on a 40-hr work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health
Shifts available: Days
Shift timings: 8 Hours; Days, 8:00 am - 5:00 pm
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, No weekends or On-call
Position Requirements:
- LPN experience: 1 - 2 years - Required
- Recent/Current Home Health Nursing experience: 1 year required and preferably within the last 3 years
- Home Care Home Base (HCHB) charting - Preferred
- Must have valid Driver’s License and Personal Auto Insurance
Education, License, and Certification Requirements:
- LPN License for State of the state applying or Compact if applicable - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description:
- Provide skilled nursing care as ordered by the attending physician under RN's supervision.
- Observe and monitor patient conditions and notify RN case manager of any changes in the patient’s condition.
- Administer medication as prescribed by the physician under RN's supervision and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions.
- Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
- Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
- Attend IDT Team meetings
- Territories to cover: North and Northeast of King County
