Location: Private Care Facility, Pomona, CA
Schedule: (Female Preferred) 12-Hour Shifts | Rotating Weekends | Long-Term Ongoing Case

Position Overview
We are seeking a compassionate and reliable Licensed Vocational Nurse (LVN) to provide dedicated one-on-one care for a male client who sustained a catastrophic work-related injury on December 15, 2023. Following a steel beam accident, the client suffered a traumatic brain injury (TBI) and cervical spinal cord injury (SCI) resulting in quadriplegia.
This is a long-term, ongoing case in a private facility in Pomona, CA, where the LVN will provide consistent care and build a supportive, therapeutic relationship with the client. The assignment is considered straightforward and rewarding, as the nurse will focus exclusively on one patient in a stable environment. This is not your typical nursing assignment. You will be providing dedicated, one-on-one care to a single patient in a supportive environment. The client is a male with quadriplegia following a work-related injury. He is stable, has all necessary equipment in place, and requires primarily catheter care and long-term nursing oversight.
Unlike facility floor nursing with heavy workloads and multiple patients, this role allows you to truly focus on one individual, build a connection, and provide quality care without feeling rushed.

Client Care Details
- Diagnosis/History:
- Quadriplegia due to TBI and cervical SCI
- Multiple neck injuries
- Wheelchair- and bed-bound
- Durable Medical Equipment (DME) Provided:
- Hospital Bed
- Wheelchair
- Hoyer Lift (preferred transfer method)
- Transfer Board (secondary option)
- Shower Chair
Key Responsibilities
- Provide ongoing catheter care: insertion, maintenance, monitoring, and infection prevention.
- Manage bowel and bladder program while monitoring for complications such as UTIs or retention.
- Perform routine nursing assessments and monitor patient condition, including vital signs.
- Assist with safe transfers and repositioning using the Hoyer lift, transfer board, and other equipment.
- Support ADLs (Activities of Daily Living) such as hygiene, comfort care, and mobility assistance within LVN scope.
- Maintain accurate documentation and charting of care provided and changes in condition.
- Collaborate with supervising RN/MD, therapy teams, and family members for continuity of care.
- Ensure infection control and skin integrity management through proactive repositioning and monitoring.
- Provide compassionate emotional support while respecting patient dignity and independence.
Qualifications
- Active, unencumbered California LVN license.
- Minimum 1 year of nursing experience (long-term care, rehab, or SCI patients preferred).
- Proficiency in catheter care and bladder management programs (required).
- Experience with quadriplegia or spinal cord injury patients highly desirable.
- Must be able to work 12-hour shifts with rotating weekend availability.

Job Type: Full-time
Pay: $33.00 - $35.00 per hour
