We are seeking a Provider Relations Clinical Liaison that will provide education about services to medical providers within assigned territory and meet immediate admission needs of patients.
Schedule:
Monday - Friday; 8:30am - 5:00pm

Essential Duties & Responsibilities include (but are not limited to):
- Identifying, establishing and maintaining positive relationships with referral sources, including including physicians, hospitals, healthcare providers, community facilities, senior housing, and community service agencies to ensure ongoing quality of care and referrals
- Assist in determining eligibility for home health care
- Assist with the implementation and coordination of services required for the discharge of patients from hospitals into the agency’s home health program
- Maintain close communication with intake personnel and regarding home health referral or admission
Education/Experience:
- Associate or bachelor’s degree from an accredited school
- Minimum of one-year professional experience in an acute care or home health care setting.
- Experience in a home health or hospice setting is preferred but not required
Certificates, Licenses, Registrations:
- Current license as a registered professional RN, PT, OT in the State of California.
Pay Range:
$64.00 - $72.00 hourly
