Legacy Home Health & Rehab, is currently seeking experienced and compassionate Registered Nurses to join our growing team. We are hiring for both full-time Home Health Admissions RN positions and Per Diem RN roles.
These positions will serve patients located throughout East County, including but not limited to, El Cajon, La Mesa, Santee, Lemon Grove, Lakeside, Spring Valley, Alpine, Jamul, and Julian.
Qualified candidates should have a strong understanding of home health care protocols, with working knowledge of OASIS-E1 documentation and PDGM (Patient-Driven Groupings Model) guidelines. The ideal nurse is comfortable working independently in the field, has excellent clinical judgment, and is committed to providing high-quality, patient-centered care in the home setting.
Responsibilities:
- Conduct comprehensive nursing assessments, including all required OASIS assessments (admission, transfer, resumption of care, re-certification, and discharge).
- Evaluate the patient’s condition, home environment, and available caregiver support to determine if the services of a Home Health Aide will be required and the frequency of this service.
- Develop, implement, and regularly update individualized Patient Care Plans in collaboration with the attending physician.
- Monitor and evaluate the effectiveness of nursing care on an ongoing basis, making necessary adjustments to meet patient needs.
- Participate in case conferences and communicate with supervisors about patient issues and care strategies.
- Engage in the discharge planning process to ensure smooth transitions and appropriate follow-up care.
- Coordinate care with other healthcare providers and community resources to support a comprehensive care plan.
- Provide education, supervision, and counseling to patients and caregivers regarding treatment, medication, and self-care.
- Supervise and evaluate the performance of Home Health Aides, at least every 14 days or as needed.
- Ensure all clinical documentation (visit notes, progress reports, etc.) is completed accurately and submitted within 48 hours.
Qualifications:
- Currently licensed as a Registered Nurse (RN) in the state of California.
- Minimum of one year of nursing experience preferred; background in home health, community care, or medical/surgical settings is a plus.
- Demonstrates compliance with accepted professional standards and nursing practices.
- Strong verbal, written, and organizational communication skills.
- Must hold a valid driver's license, have access to a reliable vehicle in good working condition, and carry auto insurance per California and agency requirements.
- Maintains current BLS certification (hands-on CPR required).
- Proficient in wound care, including experience with wound vacs, Foley catheter care, and ostomy care.
- Familiarity with Kinnser/WellSky EMR systems preferred.
