The Hospice Field Nurse is responsible for providing skilled nursing care and supportive services in accordance with the interdisciplinary plan of care for terminally ill patients in their homes or facilities. The Field RN ensures symptom management, patient and family education, and emotional support while upholding hospice philosophy and Medicare Conditions of Participation.
Responsibilities
- Perform comprehensive assessments and re-assessments of patients’ physical, emotional, and psychosocial needs.
- Develop, implement, and regularly update individualized care plans in collaboration with the IDG.
- Provide direct nursing care, including pain and symptom management, wound care, medication administration, and coordination of DME and supplies.
- Educate patients, families, and caregivers regarding disease process, symptom management, medications, and hospice philosophy.
- Maintain accurate and timely electronic documentation per agency policy and CMS regulations.
- Communicate effectively with physicians, pharmacists, and interdisciplinary team members to ensure continuity of care.
- Participate in on-call rotation as scheduled, providing after-hours visits and support when needed.
- Attend and actively participate in interdisciplinary group meetings.
- Advocate for patients’ comfort and dignity, honoring end-of-life wishes and cultural/spiritual preferences.
- Comply with all applicable federal, state, and local laws and hospice policies.
Job Type: Full-time
Pay: $80,000.00 - $90,000.00 per year
