Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. Duties
- Assess and evaluate patients' needs to develop individualized care plans.
- Coordinate with healthcare providers, patients, and families to facilitate effective communication and care transitions.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide education and support to patients regarding their health conditions and treatment options.
- Maintain accurate documentation of patient interactions, care plans, and progress notes.
- Collaborate with interdisciplinary teams in various settings
- Utilize knowledge of medical terminology and case management principles to enhance patient care delivery.
- Advocate for patients' needs within the healthcare system while ensuring compliance with policies and regulations.
Qualifications
- Current Registered Nurse (RN) license in good standing.
- Experience in hospice care is preferred.
- Previous experience in case management is highly desirable.
- Strong understanding of medical terminology and healthcare systems.
- Excellent communication skills, both verbal and written.
- Ability to work collaboratively within a multidisciplinary team environment.
- Strong organizational skills with the ability to manage multiple priorities effectively.
