Overview
We are seeking a compassionate and dedicated Hospice Care RN to join our team. The ideal candidate will play a critical role in providing high-quality end-of-life care to patients and their families. An RN with hospice, home health or long term care experience is preferred , with a focus on patient-centered care, effective communication, and interdisciplinary collaboration.
Responsibilities
- Provide hospice services for patients, ensuring comprehensive care plans are developed and implemented.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to provide holistic support to patients and families.
- Conduct assessments to evaluate patient needs and preferences, including pain management and behavioral health considerations.
- Monitor patient progress and adjust care plans as necessary, utilizing evidence-based practices.
- Provide education and support to patients’ families regarding hospice services, symptom management, and end-of-life issues.
- Ensure compliance with regulatory requirements and maintain accurate documentation of patient care activities.
- Participate in quality improvement initiatives to enhance service delivery and patient outcomes.

Pay: $35.00 - $52.50 per hour
