The Licensed Clinical Hospice Social Worker helps clients, and their families navigate planning for end-of-life care; understand their treatment plan and be vocal about their needs; manage the stressed of debilitating physical illnesses, including emotional, familial, and financial; overcome crisis situations; and connect to other support services. They are advocates for patients and their families and know what their clients need and what resources are available to help them.
- Collaborate with IDT Team to create care plans to meet the needs of individual patients and families.
- Communicate with patients and family as well as Skilled Nursing Facility Staff, Hospital Staff and other patient providers and caregivers to provide the best care possible.
- Compassion a must.
- Document timely and accurately
- Ability to think creatively and work with care team and family members to treat patients the most effective way possible.
