Salary: $26.46– 39.50 per hour, payable bi-weekly. The salary scale for this position is between steps 0 and 19, depending on experience. This is a non-exempt, 40 weekly hours. Applicants must pass an extensive background check.
Job Summary:
The Housing Case Manager supports clients eligible for Ryan White HIV/AIDS Program services by helping them secure and maintain stable housing. They will be familiar with or learn Seattle Roots HIV case management services, including client responsibilities, documentation requirements, and program benchmarks. This role involves providing support from the initial referral through housing placement, stabilization, and retention, while addressing barriers that could jeopardize housing. Using harm reduction and Housing First approaches, the Housing Case Manager works with individuals who may have mental health challenges or use substances, offering low-barrier access to housing and services. They assist clients in setting and achieving goals related to housing, financial stability, health, and personal growth, and connect them to essential resources such as healthcare, employment, and basic needs. The position also includes supporting clients in transitioning to long-term or self-supported housing to ensure positive outcomes and free up program capacity for others. This position requires meeting clients in the field and may require up to 50% of time working outside of the office setting. This position requires reliable transportation and an unrestricted driver's license.
Duties/Responsibilities:
- Provide engagement and case management for clients referred for Ryan White Housing Services, building rapport to support self-sufficiency and housing stability
- Understand and apply eligibility criteria, housing program rules, application processes, and move-in assistance protocols
- Maintain working knowledge of local housing issues and landlord-tenant laws in Seattle/King County
- Develop and update individualized housing plans, addressing barriers to housing, income, health, and personal growth
- Assist clients with housing applications, paperwork, and placement, including support for those with disabilities or other housing challenges
- Provide continued support for newly housed clients, promoting stabilization and retention through advocacy and intervention as needed
- Conduct annual and interim reviews of income and rent, complete home inspections, and ensure unit safety and habitability
- Schedule and conduct home visits, at least annually or more frequently based on client need or housing issues
- Advocate for clients with landlords and property management to resolve issues, support integration, and ensure housing retention
- Connect clients to needed health, mental health, substance use, and supportive services, encouraging both direct assistance and self-advocacy
- Maintain accurate documentation for compliance and outcome tracking, ensuring confidentiality.
- Participates in any initiatives developed and implemented by the Quality Assurance Department.
