Position Summary: Provides operational oversight, professional guidance, and direction to the Care Transitions team, ensuring services are provided efficiently and effectively. Regularly reviews and revises, as necessary, relevant standards and ensures services performed comply with all hospital, system, and regulatory agency standards. Coordinates provision of services with other nursing and medical functions and serves as a technical resource for departmental personnel. As an expert in the assigned area serves as a resource person to staff and physicians.
Duties/Responsibilities:
- Assesses the patient at every clinic visit and documents any change in status in the clinical record: completes the facility super bill for each patient encounter, and keeps accurate patient records as they pertain to HBO and Wound Center treatment plans.
- Assumes responsibility for the safe and effective operation and maintenance of the HBO chambers and related systems.
- Makes necessary recommendations to the Program Manager and Medical Director for departmental safety policies and incorporate new safety procedures as appropriate.
- Provides leadership and safety training to other HBO facility personnel, including other departments.
- Helps prepare the hospital for the UHMS survey.
- Provides all patient education and patient care in the HBO unit.
- Participates in Quality assurance program and develops PDSA cycles when issues arise.
- Maintains competencies, continued education as appropriate. Maintains competencies for all HBO staff.
- Insures Order, Consent and Insurance authorization is in place prior to initiating treatment plan.
- Fosters a positive relationship between the department and local Fire Marshall and Emergency Medical Director and any other hospital departments that interface with the HBO program.
- Works closely with Program Manger to coordinate and approve all HBO chamber upgrades, modifications and repairs to the HBO system. Includes interactions with hospital maintenance and that all activities are done in accordance with appropriate safety standards.
- Maintains a safe environment in HBO room for patients and staff and uses approved and recognized mono-place treatment protocols.
- Cleans each chamber between patients.
- Facilitates monthly HBO safety drills.
- Evaluates HBO chamber equipment /supplies used in the chamber and ensures that they meet all safety requirements before they are permitted inside the chamber.

Education/Experience Required:
- Graduate - approved School of Nursing
- Ability to read, write and comprehend English at 12th grade level. Evidence of continuing education.
- Demonstrated proficiency in basic nursing care.
- Experience in Wound care and hyperbaric medicine preferred, or currently working as an LPN in Medical surgical setting, with 5+ years of experience.
Registration/Certification:
- Current LPN License in Massachusetts.
- Wound Care or Hyperbaric Certification required within a year of hire.
Physical/Environmental Requirements
This job requires frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs. There will be occasional need for climbing stairs, lifting in excess of 50 – 100 lbs and sitting. Constant use of hearing, sight and speech. Frequent use of smell and touch. Frequent keyboard and telephone use. Constant concentration and problem solving.
