We are seeking a compassionate and dedicated IHS/ Caregivers to provide essential care and support to individuals in need.
In this role you will:
- Assist with daily living activities (e.g., meal prep, home organizational support, light housekeeping, and medication reminders, etc)
- Facilitate communication between individuals and their case managers to ensure their needs are met
- Offer companionship and support in maintaining independence and overall well-being while achieving their goals
- Be working one-on-one with individuals in both home and community settings, ensuring they receive personalized care and support to improve their quality of life
- Observe and report any changes in the individual’s condition or well-being to the appropriate support team
- Encourage and support individuals in social, community, and recreational activities
- Ensure safety and security of individuals in their home and community settings.
- Maintain accurate records of care, progress notes, incidents, etc
- Work independently and manage time effectively while maintaining a strong communication link with team leads and the overall support team.
- Adhere to all organizational policies and procedures, including confidentiality standards.

Qualifications:
- High school diploma or equivalent
- Additional certifications or previous experience in healthcare, social services, or related fields- Preferred!
- Previous experience and knowledge of working with individuals with disabilities, seniors, dementia, etc
- Strong organizational skills and the ability to work independently
- Ability to perform tasks with empathy, patience, and respect
- Skills to be successful in this role: compassionate, empathetic, and reliable with a strong desire to help others
- Excellent communication and interpersonal skills to interact effectively with individuals, case managers, and team leads
- Ability to work effectively in a team
- Ability to handle sensitive and confidential information
- Must possess a valid Driver’s License and have access to a reliable and insured vehicle
- Ability to work flexible hours as needed.
- Previous experience in a similar role or direct care position preferred but not required
- Must pass a background check and comfortable with DHS fingerprinting
