What you’ll be doing:
- Answers incoming calls from potential patients/clients, healthcare providers, and other referral sources to assess eligibility for agency services.
- Receives and appropriately documents new referrals in a timely manner and completes the benefit verification process.
- Once the Clinical Manager accepts a new referral this position will be in charge of gathering medical information/records and other pertinent paperwork from referral sources to maintain comprehensive intake files.
- Participates in coordinating care with Clinical Manager, patients/clients, and the Interdisciplinary Group/Team to ensure seamless patient/client care transition.
- Provides effective communication to patients/clients, staff members, other health care professionals, and referral sources to foster and maintain positive relationships.
- Provide clerical support which may include preparation of admission, orientation, and new hire packets; monitoring and ordering office supplies and forms, typing, special projects, filing and copying.
- Facilitate the communications system which may include: mail distribution, memos, answering incoming calls, and document and disseminate appropriate messages.
- Assist with maintenance of personnel files, obtaining signed doctor orders, scheduling clinicians for client visits
- Promote Boost’s philosophy and mission by presenting a positive image to patients, families, physicians, and community agencies.
- Provide computer support and data entry.
- Provide effective communication to patient/family, team members, and other healthcare professionals and maintain confidentiality.
What we’re looking for:
- High school graduate
- One (1) year general office experience in Home Healthcare
- Excellent computer skills required
- Demonstrated interpersonal and organizational skills
- Reliable transportation and valid auto liability insurance
- A passion to serve and help others live their best lives possible.
- Active CNA license beneficial
- 41,500 to 52,000 per year
