Suburban Home Health is a joint venture between several Suburban Health Organization hospital systems in Indiana. This allows us to work as an extension of our partner hospital systems to service homebound patients in our local communities. The Intake Nurse is part of a collaborative team that ensures patients receive timely, high-quality care at home. This position offers a hybrid schedule with two days per week in the Noblesville office and the remainder remote, giving you the balance of teamwork and flexibility.
The Intake Nurse is responsible for coordinating the admission process of new patients into the agency.
- Completes pre-admission review of referrals to ensure home care appropriateness
- Communicates with physicians to ensure a seamless transition to home health to achieve continuity of patient care
- Coordinates with referral sources regarding home health care admission criteria per CMS and ACHC guidelines
- Collaborates with Business Development Liaisons to identify opportunities for the promotion of agency services within the communities served
- Obtains verbal orders for home health admission
- Establishes and maintains a positive relationship with clients and referral sources
- Generates referral/intake within the EMR
- Ensures FTF forms are obtained from referral sources for Medicare and Medicare replacement patient admissions
- Tracks and monitors cancel/non-admissions
- Works closely with the Nursing Supervisors to ensure new admissions are seen in a timely manner
- Ensures compliance with all state, federal and referral/intake regulatory requirements for home health
- Coordinates with Billing regarding insurance eligibility as part of the referral process
- Communicates with referral sources to obtain required referral documentation (orders, H&P, insurance information, etc.)
- Current licensure for License Practical Nurse in the state of Indiana
- Degree in nursing from an accredited school
- At least 2 years of nursing experience, preferably in a home health environment
- Ability to multi-task and perform job duties in a detail oriented manner
- Proficiency with computers and software applications necessary to perform job functions including but not limited to MS Office Applications and EMR programs
- Strong interpersonal, organizational and communication skills required
