Position Summary

The Intensive Case Manager (ICM) is a core member of our Integrated Recovery Program which serves participants struggling with substance abuse. The IRP program is founded on the principle of radical respect of all participants and offers Medication Assisted Treatment (MAT) as well as wrap-around support services. The ICM position provides high quality case management services to a caseload of vulnerable and high-risk IRP participants both at our primary care outpatient medical clinics and in the community. This position has a special focus on participants who are at risk of being lost to care and/or have been lost to care. This position is responsible for maintaining timely and accurate progress notes, reviewing, and following up on all CRISP notifications, and administering the Government Performance and Results Act (GPRA) Client Outcome Measures.

It is estimated that 25% of time will be spent in the community building relationships with IRP participants who are not otherwise able to attend in-person visits with Care Coordinators. Services will include providing support to patients seeking detoxification, obtaining insurance authorization for prescription drugs needed to manage the symptoms of regular opioid use, identifying community resources like inpatient detox programs, supporting patient transitions from the street to inpatient programs, working with patients to engage in group and individual treatment as determined by a collaborative treatment plan that supports individual independence from addiction and toward the achievement of better health outcomes overall.

Reportability

This position will report to the Addiction Specialist/Care Coordinator Supervisor, and by association be accountable to the Substance Use Disorder (SUD) Program Manager

Essential Duties and Responsibilities

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

  • Provide intensive case management to “lost to care” clients including, but not limited to: home visits, facilitation of detox and hospitalization, and overdose prevention;
  • Administer the Government Performance and Results Act (GPRA) Client Outcomes Measures for discretionary Programs at required intervals
  • Review consent for treatment, confidentiality, and client’s rights forms with participant
  • Assist new participants to access an appropriate level of care including detox and inpatient services
  • Provide education to consumers on how to use Narcan and facilitate monthly groups to educate family and community members on the use of Narcan
  • Complete Prior Authorizations (PAs) and work with insurance to ensure participants receive medication in a timely manner
  • Provide referrals/link participants to Mary’s Center services, i.e. mental health therapy, primary care, infectious disease care
  • Provide referrals/link participants to services at outside agencies including housing, vocational programs, entitlements, workforce development, financial literacy, etc.
  • Conduct Motivational Interviewing and reinforce treatment recommendations to maximize engagement of participant in all recovery supports
  • Assist and support clients in crisis situations including arranging transportation
  • Outreach clients to enhance engagement in treatment and “meet client where they are”
  • Participate in case reviews with other team members
  • Support team coverage or other program initiatives when needed
  • Document all contacts, referrals, and other necessary information accurately and in a timely fashion (within 48 hours of service provision)
  • Provide logistical support to program activities, including weekly parent support group, recovery celebrations, and community events.
  • Understand and apply state and federal confidentiality regulations with regard to participants’ records and correspondence
  • Demonstrate and develop cultural competency through appropriate trainings and other activities as needed
  • Serve as a liaison to other community organizations that service individuals with substance use disorders and represent Mary’s Center at community events

Qualifications

Knowledge, Skills & Abilities

  • High School Diploma or GED required.
  • Must have a minimum of three years working in a healthcare setting, previous experience with substance abuse or mental health preferred.
  • The ability to communicate proficiently in English is necessary.
  • A thorough understanding of privacy and confidentiality regulations is essential

Language Requirements – Ability to communicate effectively in English and Spanish is required. Additional language proficiency or fluency preferred.

Physical Demands – Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.

Work Environment – Mostly in a primary care outpatient medical clinic and in the community when meeting participants at their homes and other locations in the community. May be exposed to extreme cold, heat, and humidity due to outside weather conditions. Opportunity to work a hybrid schedule – remotely & onsite, available as needed.
