Overview
We are seeking a compassionate and dedicated LPN to join our team! The ideal candidate will provide essential support to residents, ensuring their comfort, safety, and well-being. This role is crucial in delivering high-quality care to individuals with varying needs, including those requiring memory care and assistance with daily activities. The LPN will work closely with residents, their families, and other healthcare professionals to create a supportive environment that promotes dignity and independence.
Covered Bridge is small 12-Bed Assisted Living Facility, and we are looking to add 1 part-time LPN to our team!
Pay is $24.00/hr. with a shift differential for weekends.
Currently hiring part time with flexible hours, Monday thru Friday.
Weekends are scheduled as needed.
JOB SUMMARY:
The LPN performs a variety of professional nursing duties in the care of assisted living residents, complies with state rules and regulations, and follows established policies and procedures of the facility.
RESPONSIBILITIES:
- Works under the direction of the RN to monitor residents’ needs and changes in condition, conducting focused assessments when appropriate and reporting the results to the RN.
- Follows resident care plans/ service plans developed by the RN and consults with RN about resident conditions and their treatments.
- Orders, sets up and administers medications and treatments per Covered Bridge Policies and Procedures.
- Assists RN in orienting, training and mentoring new staff in a professional manner.
- Performs delegated tasks and other duties as assigned by the RN.
- Assisting with ambulation and transfers, including proper use of a transfer belt and assistive devices and proper body mechanics to avoid injury.
- Assist in use of splints, braces, catheter and wound care appliances for residents.
- Assisting with resident transfers using transfer belt, EZ Stand or EZ Lift and 2 people assist.
- Assist resident with AROM, PROM, group exercises, walking programs with adaptive equipment as needed.
- Assist management team with notes, reports, filing documents, and other related office work as needed.
POSITION SPECIFICATIONS:
- Graduate of an accredited school of nursing for Licensed Practical Nurse.
- Must be registered and licensed with the State of Minnesota as an LPN, and must maintain current unencumbered license with the Board of Nursing.
- Must conduct focused nursing assessments per Nurse Practice Act of 7/2014.
- Must have a current Minnesota Driver’s License.
- Must have basic computer experience and/or be willing to learn Eldermark Software
- Must demonstrate leadership skills, time management, organizational skills, good judgement and dependability.
