Licensed Practical Nurse (LPN) – Skilled Nursing / Long-Term Care

Shift: Days(3x12)
Position Summary:
We are seeking an experienced Licensed Practical Nurse (LPN) for a skilled nursing position in our Long-Term Care (LTC) unit. This is an October-start contract role in a supportive facility where LTC is connected to our Critical Access Hospital (CAH), providing additional resources for patient care.
Assignment Details:
- Shift: 12-hour days, 6:00 AM – 6:00 PM
- Patient Load: Maximum of 17 LTC residents
- Staffing Model: Typically 1 aide and 1 nurse per shift, with ED staff available for additional support
- Setting: Long-Term Care attached to Critical Access Hospital (17 designated LTC residents)
- Scrubs: No color requirement (must be clean and professional)
- EMR: Athena
Requirements:
- Active LPN License – Required
- BLS Certification – Required
- Minimum 2 years of skilled nursing or LTC experience – Required
- Strong patient care skills and ability to work independently and collaboratively
Job Types: Contract, Travel nursing
Pay: $45.00 - $48.00 per hour
Expected hours: 36 per week
