Overview
The Wound Care LPN will assist the Nurse Practitioner in our growing mobile wound care practice. Both LPN and NP travel together to patients homes providing excellent wound care to Patients. Must have knowledge in wound care and must enjoy in home care.
Duties
- Assist Nurse Practitioner in Conducting thorough assessments of patients' wounds and overall health status
- Educate patients and their families on wound care techniques, prevention strategies, and self-management practices.
- Collaborate with healthcare professionals to coordinate comprehensive care for patients with complex needs.
- Monitor patient progress with Nurse practitioner and adjust treatment plans as necessary based on clinical evaluations.
- Document all assessments, interventions, and outcomes accurately in electronic health records (EHR).
Skills
Candidates for the Wound Care Nurse position should possess the following skills:
- some experience in home care
- strong background in Wound care
- Familiarity with discharge planning processes within hospital medicine settings.
- Knowledge of behavioral health considerations that may impact wound healing and patient compliance.
- Excellent communication skills for educating patients and collaborating with healthcare teams.
- Strong critical thinking skills to assess complex situations and make informed decisions regarding patient care.
