Overview
We are seeking a dedicated and compassionate Intake Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring that patients receive the highest quality of care. This position requires strong organizational skills, attention to detail, and the ability to communicate effectively with patients and healthcare professionals alike. The Intake Coordinator will be responsible for managing patient intake processes, coordinating care, and facilitating communication between patients and medical staff.
Responsibilities
- Conduct initial assessments of patients to gather pertinent medical history and current health concerns.
- Coordinate patient intake processes, ensuring all necessary documentation is completed accurately and efficiently.
- Collaborate with clinical and administrative staff to schedule initial assessments and start-of-care visits.
- Maintain accurate records in compliance with regulations
- Facilitate communication between patients, families, and healthcare teams to ensure seamless transitions of care.
- Has worked with electronic health record (EHR) systems previously and is technologically inclined.
- Enter and maintain accurate patient information in the EHR system.
- Assist with medical record management, including filing and organizing documents.
- Track certifications, recertifications, and face to face documentation.
- Assists with orders management.
- Assists with wellness calls.
- Order supplies.
- Work under supervision of DON.
- Other duties as assigned.
- Handle sensitive patient and staff information with confidentiality and discretion.
Experience
- LVN required.
- Previous experience in a hospice setting is preferred, particularly in roles involving patient intake or coordination.
- Strong interpersonal skills with the ability to empathize with patients and their families during challenging times.
- Ability to work collaboratively within a multidisciplinary team while maintaining a high level of professionalism.
- Strong computer and data entry skills.
