Job Description
North Houston Transitional Care is a new facility in North Houston, TX. The facility is licensed for 70 Medicare beds. We are looking for someone who is interested in making this facility the premier transitional care facility in Houston that offers shorty stay rehabilitation for patients that need therapy services after a joint replacement, surgery, or need wound treatment, IV therapy and other assistance to return home quickly. There are resort style amenities offered such as a salon, room service, fine dining, and many others - while being taken care of by a professional medical staff.
The candidate will assist with ensuring that documentation in the center meets federal, state and certification guidelines. The Care-plan Nurse will coordinate the RAI process assuring the timelines of CAAS and Interdisciplinary Care Plan.
Duties:
- Maintain a system by which new admissions, annual reviews, and quarterly reviews are to be initiated and completed within the time limit.
- Conduct observations and interviews as well as evaluations required for MDS and/or care plan preparations.
- Complete the Minimum Data Set(MDS) within 7 days of admission in coordination with other members of the inter-disciplinary team.
- Review orders with the appropriate nurse daily or more frequently if necessary.
- Provide direct resident/patient care as needed.
- Review accidents/incidents and critically ill/unstable residents.
- Review new Admissions/Medicare Specialized therapies, i.e., IV therapy and Pressure Sores, Stage 3 and 4.
- Review and assess resident's condition upon a reported accident/incident within 24 hours.
- Complete documentation and report to the Director of Nursing.
- Ensure all areas, which are triggered on the resident Assessment Protocol Summary, are included on the resident's plan of care.
- Must maintain employee/resident/patient confidentiality at all times.
QUALIFICATIONS:
- Graduation from an accredited School of Nursing with a Bachelor of Science Degree in Nursing (BSN preferred).
- Current registered Nursing License (RN) or LVN/LPN License.
- Work experience in a skilled nursing facility is required.
- Two (2) years MDS Nurse experience preferably in long-term care facility.
- Knowledge of RAI Process and experience with PCC preferred. CPR certificate.
- Thorough understanding of the Quality Indicator process
- Knowledge of the OBRA regulations and Minimum Data Set
- Knowledge of the care plan process
PHYSICAL REQUIREMENTS: Have the ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching and lifting up to 75 pounds with or without reasonable accommodations
WORK SCHEDULE: Includes some weekends, evenings and holidays
