Position Overview:
The MDS Coordinator Including Special Projects as assigned, is responsible for coordinating the Minimum Data Set (MDS) process to ensure accurate and timely submission of resident assessments in compliance with federal and state regulations. This role supports optimal reimbursement and care planning through detailed documentation and interdisciplinary collaboration.
Key Responsibilities:
- Coordinate and complete timely and accurate completion of all MDS assessments in accordance with federal and state regulations and organizational guidelines.
- Accurately collect, enter, and submit MDS data within mandated timeframes.
- Collaborate with interdisciplinary team members to ensure completeness and accuracy of assessments, comprehensive documentation of resident care and care plans.
- Monitor assessment schedules to ensure timely submissions and avoid late penalties.
- Audit medical records to ensure coding accuracy (e.g., RAI process, ICD-10).
- Develop and update individualized care plans based on MDS assessments and clinical input.
- Monitor and audit clinical documentation to ensure consistency and accuracy.
- Educate and train staff on MDS procedures, documentation standards, and regulatory compliance.
- Ensure compliance with Medicare, Medicaid, and managed care reimbursement guidelines.
- Maintain up-to-date knowledge of regulations impacting MDS processes and long-term care reimbursement.
- Attend care plan meetings and participate in surveys and inspections as needed.
- Other duties as assigned.
Qualifications:
- Current Registered Nurse (RN) license in good standing, licensed in NH
- 3 Years’ Experience in Long Term Care
- RAC-CT certification (Resident Assessment Coordinator–Certified) from AANAC
- Bachelor’s degree in nursing or related field
- Outstanding Computer skills and knowledge
- Proficient in MDS software (We have PCC - Point Click Care)
- Strong written and verbal communication skills
- Detail-oriented with excellent analytical and organizational skills
Working Conditions:
- Primarily office-based within the facility with occasional floor time and audits.
- May be required to work off-hours or weekends during surveys or special initiatives.
- Standard office environment within a healthcare facility
- Requires occasional lifting, walking, and standing
- May involve weekend or after-hours work for urgent compliance needs
