Under direct supervision and in accordance with established policies and procedures, assists with planning, implementing, and evaluating patient care. This role is also responsible for the smooth function of patient flow.
Job Responsibilities
- Educating patients on Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and other telehealth programs
- Inputting and reviewing patient data in the RPM, CCM, and other telehealth programs in the dashboard
- Monitoring patient vital signs, such as pulse rate, temperature, blood pressure, and weight and records information in patient records and creating care plans for them
- Engaging patients by phone, text, or possibly video to: Remind patients to record their vitals; Respond to alerts related to patient symptoms or physiologic data; Respond to patients for minor technical troubleshooting
- Reviewing the vitals, communication of results, and patient concerns to MD/NP as appropriate
- Performing other duties as assigned
Education
- High School Diploma
Experience
- Graduate/ B.Pharma
- Medical Assistant Certification
- Entry-level or some experience in the medical office practice setting
Knowledge, Skills, and Abilities
- Demonstrated strong communication and interpersonal skills; demonstrated ability to interact with multiple constituencies and exercise "people skills"
- Demonstrates ability to exercise standards of professionalism, including appearance, presentation, and demeanor
Job Type: Full-time
Pay: $19.00 - $21.00 per hour
