The Chronic Care Coordinator performs care management for chronically ill patients with chronic diseases such as chronic kidney disease, diabetes mellitus, chronic obstructive pulmonary disease, and/or congestive heart failure. The Chronic Care Coordinator works in collaboration and continuous partnership with chronically ill patients and their family/caregiver(s), clinic providers in a team approach to increase patients’ ability for self-management and shared decision-making
Duties
- Responsible for registry of chronic care management (CCM) patients.
- Validates enrollment of CCM patients based on provider request
- Conducts minimum of one 20 minute of telephone and education per month to each CCM patient on roster.
- Complies with documentation requirements of the Chronic Care Management program by carrying out the care plan with the patient, family/caregiver(s) and providers and recording in the EHR.
- Monitors adherence to care plans, evaluates effectiveness, monitors patient progress on time, and facilitates changes as needed.
- Medication reconciliation with review for adherence
- Reinforce disease self-management education and preventive care guidelines, communicate PCP instruction and advice, and provide patient education materials
- Communicate with and assist patients in a professional and courteous manner.
-Work as part of the healthcare team to assess patient needs, plan and modify care and implement interventions. Communicate with the healthcare team to provide a proactive approach to patient care.

The position is mainly remote but occasional visit to the office may be required a few days a week.
Certification and Licensures:
- CMA or RMA certification or LSW or LPN or RN license required
