Job Summary
Under the direction of Clerical Supervisor/Coordinator, transcribes reports on patients as dictated by physicians, maintaining acceptable degree of production and quality. Performs other office procedures as needed. The scope of care includes age groups ranging from newborn to elderly.

Job Specific and Unique Knowledge, Skills and Abilities
- Aptitude for correct spelling and accurate of medical terms and grammar
- Superior typing skills at 65 WPM required, 70-80 wpm preferred on a personal computer
- Proficient use of dictating/transcription equipment
Essential Tasks and Responsibilities
- Transcribe reports for area of specialty (Imaging, Lab Stribling Heart Center) from dictation in an accurate, neat and timely manner maintaining 95% typographically error-free reports as evidenced by random audits by supervisor. Produce acceptable volume of transcription up to and exceeding standard production rate (radiology: 250 lines of dictation per hour). Proofread all typed Imaging/Lab/Stribling reports and other transcription, revising and changing as necessary to ensure accuracy with less than 5 returned documents per month. Includes insuring dictated exam matches ordered exam. Calls/Faxes/Distributes reports as required in a timely manner.
- Maintains open communication with Clerical Supervisor, system administrator as well as other departments to facilitate accurate and timely patient records and to assist in retrieving information. Observes appropriate channels of communication and line of authority. Verifies time and attendance entries with Operations Manager bimonthly.
- Responsible for maintaining wide knowledge base of medical terminology to facilitate transcription of reports from radiologists’ dictation. Shares with other transcriptionists knowledge of new, unusual terminology or macros as indicated by observation of supervisor.
- Performs special projects and other duties as assigned. Assists with maintaining overflow transcription to keep backlog to a maximum of two (2) hours. Attends department meetings (minimum of 10 out of 12). Participate in training of new staff. Performs other administrative duties as assigned.
Area/Unit Specific Essential Tasks and Responsibilities
- LAB - Dept. 122. Demonstrates thorough knowledge of Snomed coding, maintains up-to-date medical terminology for reporting, and expert knowledge of tissue procedure dictionaries. Demonstrates through knowledge of “sign-out” for corrections or necessary changes. Performs accurate transcription with no more than two documented errors per year. Responsible for maintaining quality assurance manual, policies and procedures for state inspectors through use of Snomed coding, and data searches to maintain Laboratory accreditation. Responsible for sending/receiving pathology reports, slides, mailing, and recording in APQ section of SunQuest’s QA reporting function. Responsible for pulling slides from storage, previous reports for pathologist’s review, checking to make sure all information is complete. Responsible for back up for Customer Service Department, back up for Cytotech, back up for Clerical Coordinator, and miscellaneous clerical duties. Providing patient results, specimen requirements, and miscellaneous information to various physician offices, area hospitals, and patients for Customer Service Department. Entering pap smear results for Cytotech to be reviewed by Pathologists. Schedule paternity’s, bone marrow, FNA’s and miscellaneous pathology procedures. Pulling and preparing Tumor Board cases for Pathologists to present every other Wednesday. Back up Clerical Coordinator by distributing lab mail, distribution of reports to courier, ordering lab supplies, purchasing, and miscellaneous supplies.
- DIAGNOSTIC IMAGING - Dept. 165. Notify dictating radiologist of any “questionable” dictation by clearly annotating report with red highlighted message and placing report on “Hold” until resolved. Includes following up on reports placed on “Hold” until resolved. Maintains close communication with RIS administrator regarding areas of concern and issues (i.e., credits, system performance, etc.) and Clerical Supervisor regarding exams not completed, order entry errors, etc. Compliance in accurately and completely utilizing RIS according to department standards (i.e., maintaining dictionaries, macros, etc.), and CPAC procedures when indicated.
