Overview
The Medical Social Worker plays a vital role in the healthcare system by providing support and resources to patients and their families. This position involves working with individuals facing various medical challenges, including those related to geriatrics, autism, and hospice care. The Medical Social Worker will assess patients' needs, develop care plans, and facilitate access to community resources while ensuring that emotional and social aspects of health are addressed.
Duties
- Conduct comprehensive assessments of patients' social, emotional, and environmental needs.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Provide case management services to ensure continuity of care for patients.
- Offer clinical counseling and behavioral therapy to patients and families as needed.
- Assist patients in navigating healthcare systems, including hospital services and community resources.
- Engage in crisis management to support patients during critical situations.
- Collect and analyze data related to patient outcomes and service effectiveness.
- Work with children experiencing autism, providing tailored support strategies.
- Collaborate with interdisciplinary teams to enhance patient care quality.
Skills
- Strong case management skills with the ability to coordinate multiple services for patients.
- Experience working with children, particularly those on the autism spectrum.
- Proficient in data collection methods for assessing patient needs and outcomes.
- Knowledge of geriatric care practices and challenges faced by elderly patients.
- Expertise in clinical counseling techniques to address mental health issues.
- Familiarity with behavioral therapy approaches for various patient populations.

Pay: $95,000.00 - $125,000.00 per year
