Job Description:
We are seeking a dedicated and detail-oriented Care Navigator / Referral Coordinator to join our healthcare team. This is a mobile position requiring daily travel to screening sites across Escambia and Santa Rosa or Okaloosa and Walton counties using your personal vehicle (mileage reimbursement provided). Responsibilities include setting up and breaking down screening sites, coordinating patient referrals, and ensuring individuals receive timely and appropriate care through our community-based health screening program.
The ideal candidate will have strong communication skills, experience in patient navigation or case management, and a solid understanding of healthcare systems. Prior experience in public health is preferred.
Key Responsibilities
- Referral Coordination & Case Navigation
- Assist individuals in accessing appropriate healthcare services, including specialists, primary care providers, and chronic disease management programs.
- Ensure all necessary referral documentation is complete and accurate.
- Work closely with healthcare providers, insurance companies, and community organizations to facilitate seamless transitions of care.
- Conduct patient intake and assessments during mobile health screenings.
- Provide one-on-one case management to support clients in overcoming barriers to care (e.g., transportation, financial constraints, lack of insurance).
- Maintain accurate patient records in compliance with HIPAA regulations.
- Schedule and coordinate mobile health screening events in collaboration with community partners.
- Assist in marketing and outreach efforts to raise awareness of available health resources.
- Track and report client outcomes, referral success rates, and overall program effectiveness.
Medical Records & Coding (Preferred, Not Required):
- Support the management of medical records related to referrals.
Qualifications & Skills
Required:
- Experience in case management, patient navigation, social work, or a related healthcare field.
- Strong understanding of community resources, healthcare systems, and social determinants of health.
- Excellent organizational skills with the ability to manage multiple tasks simultaneously.
- Strong interpersonal skills, demonstrating empathy and professionalism when interacting with patients and healthcare professionals.
- Ability to work independently as well as part of a team in a fast-paced environment.
- Valid driver’s license and access to a reliable personal vehicle for daily travel (mileage reimbursement provided).
Preferred but Not Required:
- Experience in a public health setting.
- Experience with electronic health records (EHR) systems.
- Bilingual (Spanish/English) is a plus.
- Coding experience is beneficial but not required.
