The correct candidate will have at least 1 year of experience performing the following duties:
- Assessing patient needs while delivering quality care and treatment while achieving optimal patient outcomes for patients with chronic kidney disease and ESRD patients.
- Monitors and documents patient condition on a ongoing basis and responding to the care plan
- Communicate with Nephrocare team about patient condition or patient updates on a consistent basis
- Assuring that patient need for immediate and/or emergent care is handled promptly
- Collaborates well with all levels of the clinical team
- Understands value based care and principals to practice
- Willingness to slow progression and minimize complications, while keeping the patients ideals in mind
