Nurse Practitioner
Schedule: Full-time
Reports to: Associate Director of Medicine
FLSA Status: Exempt
A dynamic and inclusive community health center is seeking a Nurse Practitioner to provide comprehensive primary care services and support health promotion efforts. This role offers the opportunity to serve a diverse patient population, including immigrants and underserved communities, within a collaborative care model.
Key Responsibilities:
- Provide ongoing primary care to adult patients, including evaluation and management of acute and chronic conditions.
- Perform initial assessments of mental health issues such as anxiety and depression.
- Order and interpret diagnostic tests using evidence-based practices and high-value care principles.
- Prescribe medications and treatments within scope of licensure.
- Counsel patients on treatment plans, preventive screenings, and lifestyle changes.
- Refer patients to specialists as needed.
- Maintain thorough and timely documentation in the electronic medical record (EMR).
- Participate in call coverage, clinic meetings, quality improvement initiatives, and interdisciplinary collaboration.
- Engage in patient outreach and site health promotion activities.
- Stay current with certifications and continuing education.
- Comply with all healthcare laws, including HIPAA and mandated reporting.
Qualifications:
- Master’s or Doctoral degree in Nursing (MSN, DNP)
- Graduate of an accredited Nurse Practitioner program
- Certified as a Family or Adult Nurse Practitioner
- Active NYS NP license and DEA registration
- Current BLS and infection control certification
- Strong interpersonal and communication skills
- Experience or comfort working with diverse, immigrant, and underserved populations
- Proficiency in Spanish or an Asian language preferred
- AAHIVM and/or LGBT health certification is a plus
Job Type: Full-time
Pay: $115,000.00 - $125,000.00 per year
