Duties
- Conduct thorough patient assessments and evaluations to determine health status.
- Develop and implement individualized treatment plans based on patient needs.
- Perform procedures such as venipuncture and infection control measures.
- Provide urgent care services as needed, ensuring timely intervention for acute conditions.
- Collaborate with interdisciplinary teams to coordinate patient care, including memory care and senior care services.
- Utilize electronic health record systems such as Cerner for documentation and patient management.
- Educate patients and their families about health management, preventive care, and disease processes.
- Monitor patient progress and adjust treatment plans as necessary.
- Maintain compliance with all healthcare regulations and standards.
- Graduate Nursing or related field from an accredited program.
- Current Nurse Practitioner license in the state of practice.
- Experience in gastroenterology, geriatrics, or internal medicine is preferred.
- Proficiency in performing venipuncture.
- Strong knowledge of infection control practices within a clinical setting.
- Excellent communication skills with the ability to build rapport with patients and families.
- Ability to work effectively in a fast-paced environment while maintaining attention to detail.
