Today we are building a full stack medical network that leverages our predictive capabilities to provide a frictionless experience for both our patients and care providers.
We are looking for a Nurse Practitioner (NP) to join our street medicine team, providing mobile medicine services to CalAIM populations in Los Angeles, CA. Under the supervision of a physician, the Nurse Practitioner will work independently providing assessment, management, and treatment of disease processes in patients. These patients will include people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, and adults with complex medical needs.
Candidates with experience and/or interest in public health, home care, social work or other outpatient experience working with vulnerable populations are particularly well suited for this position. As an NP on our team, you will work collaboratively with the Street Medicine Team to create and implement Care Plans, provide support, education, and advocacy for members in need, and attend case conferences as part of a multidisciplinary team.
Key Responsibilities:
- Manage a patient panel with a broad array of patient needs; conducting a mix of acute, chronic, and well visits in the field where our patients live.
- Examine patients, diagnose illnesses, provide treatment, and prescribe medication
- Gather health histories, perform physical examinations and diagnostic procedures
- Ongoing collaboration with teammates via daily huddles and weekly prioritization meetings
- Work closely with managing physician when unusual or unexpected events or problems occur with our patients
- Order, interpret, and evaluate diagnostic tests for your patient pannel
- Develop and implement treatment plans that help our patients achieve to transition our of homelessness
- Dictate and document all patient interactions so that we maintain an up to date record of the continuum of care for our multi-disciplinary teams to work from
Requirements for the role:
- Interpersonal and communication skills to effectively relate to patients, their families, members of the health care team, and the lay and professional community
- Demonstrated ability to supervise clinical support staff
- Skill in organizing work flow to accomplish team's objectives
- Ability to work independently, set goals and objectives, prioritize work activities
- Ability to determine when to function independently and when to request physician intervention
- Ability to work in a dynamic, outdoors environment (we are providing field-based care)
- Ability to travel 50-75% - Must have a valid driver's license, automobile insurance and reliable transportation
Preferred qualifications:
- Bilingual in English and Spanish strongly preferred
- 1+ years of experience with and comfort working with CalAIM populations of focus (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs).
License, certification, and registration requirements:

Physical Demands: Most work will be in the field working with CalAIM populations (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs). Will likely require substantial walking, standing, sitting in a car, and driving in various weather conditions.
Compensation:
- Annual compensation: $135,000 - $187,200. Variable based on qualifications, experience, and other interview factors.
- $15,000 signing bonus with 1 year commitment
- Up to $5,000 of relocation costs covered
