Job Description:
Essential Healthcare Services is seeking a compassionate and experienced Registered Nurse (RN) to join our team in providing after-hours hospice care for a growing hospice organization. This is an on-call position that covers evening, night, and weekend shifts. The ideal candidate will have a strong commitment to patient care, excellent clinical skills, and the ability to work independently in a hospice setting.
Key Responsibilities:
- Provide compassionate nursing care to hospice patients in their homes or care facilities, focusing on pain and symptom management.
- Assess patients’ needs, coordinate with the hospice care team, and implement appropriate interventions.
- Respond to on-call requests for urgent patient care needs from 5 PM to 8 AM (weekdays) and all-day shifts on weekends.
- Maintain accurate and timely documentation in compliance with hospice regulations and company policies.
- Communicate effectively with patients, families, and interdisciplinary team members to ensure quality end-of-life care.
- Travel within the assigned territory to provide on-site care to patients as needed.
Requirements:
- Active South Carolina RN license in good standing.
- Minimum of 1 year of hospice, palliative care, or home health experience preferred.
- Strong knowledge of pain management, symptom control, and end-of-life care.
- Excellent communication, organizational, and time management skills.
- Ability to work independently and make critical decisions in a home or facility setting.
- Reliable transportation and willingness to travel throughout the assigned service area.
Schedule:
- On-call hours: 5 PM – 8 AM Monday through Friday, and 24-hour shifts on weekends.
Compensation:
- Competitive hourly wage for on-call hours.
- Mileage reimbursement for travel.
