HOURLY RATE: $18-$22 BASED UPON EDUCATION AND EXPERIENCE

SHIFT: 1AM-9AM and ONE WEEKEND A MONTH 9PM-9AM SATURDAY AND SUNDAY

POSITION SUMMARY

Application Process ? Provide all items listed on the application requirements. Criminal records and previous work conduct cannot reflect any criminal history or misconduct that pertains to abuse, neglect, sexual exploitation, deprivation, perpetration of serious physical injury, or felony arrests. Material false statements concerning qualification requirements are not allowed and if discovered will result in immediate disposal of application. If application materials are received in satisfactory order, and the applicant meets the job qualifications, an interview will be provided for final determination.

Provide direct care and supervision to residents of the group home in accordance to with the policies and procedures provided in facility manual and Rules and Regulations for Child Caring Institutions 290-2-5. Participate in the orientation and training required. Perform duties of daily operations, and record keeping. Conduct monthly site and safety inspections. Available to parents, guardians, or representatives of children considered for admission to the institution. Conduct monthly fire drills, maintain overall general sanitary upkeep of home. Facilitate scheduled activities with residents. Notify Program Coordinator/On Call Staff immediately of any emergencies or actions conducted outside of policies and procedures. Maintain documentation in resident files on care plans and behavior management plans.

III. REQUIRED KNOWLEDGE, SKILLS and ABILITIES

1. Values working in a multicultural/diverse environment

3. Ability to work effectively with a team-oriented environment

4. Computer experience and adequate typing ability preferred

5. Ability to provide visual and auditory supervision of residents

6. Maintains appropriate professional boundaries and confidentiality

7. Regularly attend scheduled shifts, be punctual for scheduled shifts and meetings and be in a condition suitable for assuming responsibilities of the position

8. Effectively monitor safety and demonstrate group management skills and effectively deescalate potentially chaotic or volatile situations

9. Stays calm and emotionally available in an environment serving emotionally disturbed youth

10. Openness to feedback and supervision

IV. OTHER REQUIREMENTS

1. Orientation and Medication Management Training required within 30 days of employment.

2. Provides emergency first-aid and maintains a current standard First Aid/CPR certification

3. Must pass a criminal history check

4. Requires driving an agency vehicle, a valid driver?s license, personal auto liability insurance, and a driving record permitting coverage under agency auto liability policy

5. Letter from a physician stating that you able to work with children.

6. Completion of Behavior Management Training within 30 days of employment.

7. Must work one weekend shift a month from Friday 10:00 p.m. through Sunday 10:00 p.m.

8. Shift is Monday through Friday from 2:00 p.m. to 10:30 p.m.

V. SUPERVISION RECEIVED

Works under the direct supervision of the Program Coordinator.

VI. DUTIES AND RESPONSIBILITIES

1. Provides a nurturing environment and close, intensive supervision and security for female adolescent residents in the residential treatment program

i. Assures resident?s basic needs are met including meals, exercise, hygiene and sleep

ii. Teaches healthy living skills and conducts recreational and social activities enhancing skill acquisition and fun

iii. Monitors activities and whereabouts of residents

iv. Maintains a physically safe environment

v. Overseas and documents the Self-Administration of medication in an accurate and timely manner

vi. Assures that residents? daily schedule is followed

vii. Transport residents to medical, dental, counseling or other social service appointments.

2. Utilizes behavior management techniques appropriately with residents

i. Feedback is clear, specific, timely and respectful

ii. Effectively communicates behavior choices to residents, outlining the positive and negative consequences of their choices

iii. Communication is adjusted to meet the resident?s development level and style of communication

iv. Maintain accurate documentation of the Level/Point System.

v. Issues out CBC (Competency Based Consequences) appropriately.

3. Employs relationship skills effectively

i. Models healthy interactions and respectful communication with residents

ii. Forms caring therapeutic relationships that are warm, nurturing, empathetic and unconditional

iii. Relates to residents objectively and fairly, giving consideration to the resident?s individual needs

iv. Works with residents to problem solve and identify solutions to problematic behaviors

4. Works effectively within a team of co-workers

i. Maintains communication with staff members through the use of a Communication Log and shift-change meetings

ii. Tracks team and clients information in staff logs and client notes, using professional language

iii. Participates and contributes to team meetings and case reviews in a constructive manner

v. Maintains communication with co-workers throughout shift and across shifts, keeping them apprised of therapeutic issues

vi. Consults with team and follows through with decisions. Remains aware of overall program climate and assists others when need arises

5. Completes all health and safety reports, resident reports, plans and records in a timely manner in accordance with program procedures and funding contract standards

i. Write progress notes in accordance with contract standards for all individual and group skills training

ii. Write incident reports as needed and completed by shift closing.

iii. Documents all relevant contact with parents/resources, DJJ, DHR, DFCS, ORS and external agencies

iv. Completes daily shift communication and documentation at the end of each shift

v. Complete monthly site and safety reviews.

vi. Complete monthly fire, bomb threat, power failure, increment weather drills.

vii. Complete work order repair request

6. Meets regularly with Program Coordinator and/or Program Director actively participates in supervision.

7. Performs other duties as assigned by the Program Coordinator.'
