Licensed Clinical Social Worker – Palliative Care & Hospice
Full-Time or Per Diem | $45–$50/hr

Senior Doc Palliative Care is hiring a Licensed Clinical Social Worker (LCSW) to join our growing, mission-driven team. Founded by Dr. Geiss in 2014, Senior Doc has become a leader in personalized, patient-centered care. We specialize in supporting seniors, especially those with dementia, through interdisciplinary care that integrates pharmaceutical expertise to improve quality of life.

What You’ll Do:

Psychosocial Support: Assess the emotional, social, and psychological needs of patients and caregivers

Care Coordination: Collaborate with physicians, nurses, and care teams to build effective care plans

Family Education & Advocacy: Help families access resources and navigate complex care decisions

Bereavement Services: Provide post-loss counseling and support to families for up to a year

Regulatory Compliance: Maintain accurate and timely documentation in accordance with state/federal standards
What We’re Looking For:

License & Credentials: Active LCSW license required; NPI and credentialed to bill for CBT

Experience: Preferred experience in palliative care, hospice, geriatrics, or chronic disease management (including home health)

Skills: Compassionate, autonomous, team-oriented, and excellent at time management and communication

Other Requirements: Valid driver’s license and insured vehicle for patient visits as needed
