BAYADA Home Health Care has an immediate opening for a Patient Advocate Manager in our State College, PA Habilitation office. The Patient Advocate Manager will:
- Provide superior customer service and quality home care
- Focus on managing coordination of client services and emergent scheduling issues
- Manage your Patient Advocate Manager caseload while proactively growing it
- Build lasting relationships with clients, referral sources, payors and community organizations
- Develop strong, communicative relationships with the team
- Maintain effective fiscal management of your caseload by monitoring metrics and client authorizations
- Managers will partner with Clinical Managers to provide supervision and support to Habilitation Technicians
- In collaboration with the Clinical Manager, Habilitation Manager, Direct Support Professionals (DSP) and other teams to act as an advocate for clients in the Habilitation program
- Participate in Person-Centered Planning processes
- Collaborate with the client, family, Case Managers, DSPs and other teams to implement and evaluate client scheduling needs
- Participate in client ISP meetings
- Maintain ongoing communication with Case Managers/Care Coordinators and other stakeholders
Qualifications for Manager:
- Bachelor degree is required
- At least 1 year of experience with the I/DD population (Preferred)
- Prior medical office or home care experience preferred
- Prior supervisory experience a plus
- Demonstrated record of successfully taking on increased responsibility (goal achievement)
- Ambition to grow and advance beyond current position
- Strong computer skills required (electronic medical record)
- Excellent communication and interpersonal skills
