Rockbridge Area Health Center (RAHC) is seeking a dedicated Patient Care Coordinator (RN) to join our team and contribute to our mission of promoting wellness and improving health outcomes in our community.
Responsibilities include accurate documentation in electronic health records (EHR), timely facilitation of imaging and lab orders, and initiation of prior authorizations for medications and procedures. The role involves proactive patient triaging across age groups and chronic conditions, extensive patient education on disease management and preventive care, and meticulous discharge planning to ensure continuity of care. Additionally, the coordinator manages medication refills, communicates lab results promptly, and handles patient inquiries efficiently through portals and phone calls. The position also supports clinical teams as needed and performs other duties to ensure seamless clinic operations and patient-centered care delivery.
Qualifications:
- Associate Degree in Nursing (ADN) from an accredited institution required; Bachelor of Science in Nursing (BSN) preferred.
- Current RN licensure in the Commonwealth of Virginia is required.
- BLS provider certification with hands on practicum is required upon hire.
- Strong interpersonal and communication skills.
- Ability to work effectively as part of a multidisciplinary team.
3-5 years’ experience in a clinical setting preferred; OB experience preferred.
