Join Our Team as a Patient Resources Specialist at Klamath Health Partnership!
The Patient Resource Specialist offers assistance to Klamath Health Partnership’s underserved and vulnerable patients – especially those at greater risk for health disparities – in accessing social, financial and other necessary supportive services in order to coordinate the patient’s care with other professionals and improve the patient’s healthcare outcomes. The Patient Resource Specialist works closely as an ally with the patient to remove barriers to effective care by coordinating services, thus increasing the patient’s chances for a healthier quality of life.
Key Responsibilities
Patient Care
- Identify patients and help them navigate the process of accessing health coverage.
- Direct knowledge of the local community to serve as a “guide” who aids patients in accessing the health care system and other related social and financial services, including but not limited to:
- Acting as a point of contact to assist patients to obtain community-based services such as:
- Supporting community development and implementation of initiatives designed to change policies and environments to prevent chronic diseases at the local level.
- Data collection and documentation of program achievements for the purpose of internal quality improvement, grant writing, or research.
- Perform all other duties as assigned.
