Overview
We are seeking a compassionate and dedicated Registered Nurse - Home Health to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support and medical attention they need. You will work closely with patients and their families to promote health, manage illnesses, and enhance their quality of life. This position requires a strong clinical background and the ability to work independently while maintaining effective communication with healthcare teams.
Duties
- Conduct comprehensive assessments of patients' health status in their home environment.
- Develop and implement individualized care plans based on patient needs and preferences.
- Administer medications and treatments as prescribed, including IV therapy and wound care.
- Monitor patients' progress and adjust care plans as necessary.
- Provide education to patients and families regarding health management, disease prevention, and self-care techniques.
- Collaborate with interdisciplinary teams to coordinate care and ensure continuity of services.
- Document all patient interactions, assessments, and interventions accurately in electronic health records (Epic).
- Maintain compliance with all regulatory requirements and best practices in home health care.
Experience
Candidates should possess the following skills and experience:
- Proven experience in various nursing settings such as medical-surgical, clinic, emergency room, or hospital environments.
- Familiarity with infant care and experience caring for children is a plus.
- Proficiency in performing procedures such as spinal taps and suturing is desirable.
- Strong knowledge of sonography practices may enhance patient assessment capabilities.
- Excellent communication skills to effectively interact with patients, families, and healthcare professionals.
- Ability to work independently while managing multiple patient cases efficiently.
