In this role, you’ll offer compassionate support, education, and referral services by phone to birthing individuals navigating the postpartum period. As part of the Family Connects NJ program, you’ll be a trusted resource—helping families feel seen, heard, and supported. You’ll also collaborate with nurse home visitors and community members to raise awareness about perinatal mood disorders across Bergen, Essex, Hudson, Passaic, and Sussex counties, ensuring families receive the care and connection they deserve.
ESENTIAL DUTIES:
- Provide follow-up care to postpartum clients delivering and receiving care through the Family Connects NJ universal home visitation program.
- Attend weekly case conferences and provide support on mental health resources for case reviews.
- Collaborate with community/hospital-based mental health services and OB/GYN providers to facilitate the connection between patients who need perinatal mental health treatment referred through the Family Connects NJ home visitation program.
- Maintain a Perinatal Mental Health Provider Directory and listing of other relevant maternal mental health resources in Northern New Jersey, focusing on resources available in Family Connects NJ catchment areas.
- Conduct in-person and virtual education regarding perinatal mental health for Partnership nursing staff.
- Attend and maintain active participation in relevant hospital and community-based committees.
- Establish and maintain collaborative relationships with stakeholders, including public, private, and non-profit organizations, county departments, and health insurance advocacy organizations to increase awareness and support for perinatal mental health issues in Northern New Jersey.
- Complete required documentation for all activities and the program’s monthly and quarterly reports.
- Participate in community outreach and education efforts to promote services and engage with families.
- Handle other duties as requested by the Senior Manager of Public Health Programs.
JOB REQUIREMENTS:
- Bilingual preferred (Spanish, Haitian Creole, Portuguese or Arabic)
- Bachelor’s degree in social work, psychology, counseling, nursing, or a related field required
- Minimum 2 years of recent experience working in mental health required (perinatal mental health preferred)
- Knowledge of support and community resources within Northern New Jersey.
- Experience in working with culturally diverse communities/families and the ability to be culturally sensitive and appropriate.
- Excellent verbal and written communication skills.
- Excellent interpersonal and organizational skills, ability to multitask and meet deadlines.
- Computer literacy in MS Office Suite
