Key Responsibilities
- Provide physical therapy services to patients under the direction and supervision of a licensed Physical Therapist
- Assist with implementing individualized care plans to help patients restore function, improve mobility, and maintain independence at home
- Collaborate with patients, families, and interdisciplinary care teams to ensure high-quality, patient-centered care
- Accurately document treatments, progress notes, and communications in compliance with regulatory and agency guidelines
- Contribute to case conferences and team meetings to ensure effective care coordination
Qualifications
- Active Physical Therapist Assistant (PTA) license in the State of Ohio (required)
- Minimum of 1 year of recent experience as a PTA in a healthcare setting (hospital, SNF, ALF, hospice, or home health)
- Home health experience preferred, but willing to train the right candidate
- Strong communication and interpersonal skills
- Highly organized, self-motivated, and able to work independently
- Reliable transportation and ability to travel within a local territory
