Job Title: PMHNP or PA-C – Mental Health Provider for Assisted Living Facilities (Flexible Hours)

About Us:
We are an established mental health organization dedicated to alongside our private practice, providing high-quality psychiatric care to residents of reputable assisted living facilities across Suffolk County. Our team is growing, and we are currently seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant-Certified (PA-C) with experience or interest in geriatric psychiatry to join our team.
Position Overview:
The selected provider will work in collaboration with an on-site assistant to deliver compassionate, patient-centered mental health services to residents in one or two assisted living facilities per week. This is a flexible opportunity with both part-time and full-time schedules available.
Responsibilities:
- Conduct psychiatric evaluations and regular re-assessments of residents
- Provide ongoing medication management and adjustments as needed
- Collaborate with facility staff and caregivers to coordinate patient care
- Maintain accurate and timely documentation in compliance with regulatory standards
- Work alongside an assistant who will support administrative and scheduling tasks
Requirements:
- Active and unrestricted NY State license as a PMHNP or PA-C
- DEA license (or eligibility to obtain)
- Experience with or interest in geriatric psychiatry
- Comfortable working independently in assisted living facility settings
- Preferably have a medicare PTAN
- Needs to have a Medicaid MMIS number(a must have to prescribe in New York State)
- Strong interpersonal and communication skills
- Ability to travel locally within Suffolk County
