POSITION SUMMARY:
Post-Release Care Coordinators assist individuals after hours, as they transition from incarceration back into the community. This position involves transporting clients in company vehicle from Sacramento Main jail to our offices in Downtown Sacramento and/or to residential programs. Staff always work in teams of two. In some cases, staff will provide intakes assessing clients’ needs and connecting clients to services in order to promote stability, reduce recidivism, and improve long-term outcomes. The ideal candidate will have an understanding of the criminal justice system, community resources, and trauma-informed care practices.
Individuals with lived experience of incarceration, addiction recovery, and/or impacted by close family in the criminal justice system are encouraged to apply.
ORGANIZATIONAL SUMMARY:
Exodus Project launched in February 2019 as a program of St. Vincent de Paul Sacramento.
Exodus Project provides mentoring, housing, employment services, counseling, transportation, case management, micro loans, and other resources for men and women returning to the community from incarceration. The goal is to reduce recidivism, alleviate poverty, and reunify
families through a holistic approach that emphasizes human dignity, self-awareness, and an engaged spiritual life.
Through our program, thousands of returning citizens have accessed a variety of resources to support their transition home.
Exodus Project works with (and employs) individuals of all faiths and/or no faith tradition; 2/3 of our staff is formerly incarcerated.
QUALIFICATIONS:
Required
- High school diploma or GED
- Valid driver’s license and good driving record
- Other combination of education and experience will be considered
- Excellent communication, documentation, and organizational skills
- Knowledge of the criminal justice system, reentry challenges, and community-based
services
Preferred
- Case management experience
- Experience working with justice-involved populations
- Proficiency in HMIS and/or case management databases.
SKILLS AND ABILITIES:
- Appropriate, professional boundaries
- Computer skills with proficiency in MS Office
- Strong verbal communications skills
- Strong organizational skills and time management
- Understanding of the population we serve and issues related to incarceration, poverty,
mental health, and addiction
- Capable of working independently and as part of a team

HOURS AND COMPENSATION:
Full-time positions are available for evenings, overnights, and weekends.
$50,000—$52,000 annually, (FT) depending on experience and education.
